- Certified true copy of diploma
- Copy of training certificates and/or professional memberships
- Two (2) work related reference letters
- Certified true copy/notarized birth certificate
- Copy of two (2) forms of valid IDs
- Copy of valid passport (front page only)
- Copy of RN/PT license, board rating, certificate from your
- home country
- Copy of CGFNs certificate (when applicable - reimbursable)
- Copy of TOEFL with TWE certificate (when applicable/
- IELTS/TOEIC )
- Copy of TSE (when applicable)
- Copy of NBI/Barangay/Police clearance
- Criminal background check from home country
- Copy of NCLEX/FCCPT results - or Actual US License
